  8. Are you performing the work yourself, live in the home and will be living in the home 1 year after completion? *

    Homeowners can act as General Contractor - but must provide a notarized affidavit meeting the following requirements

    1. Address on application is primary residence and not a rental property

    2. Plan to live in the property one year after completion of project

    3. Hired professionals will be listed on permit

    4. Performing all work without the help of hired professionals, if so, must be listed on permit.

  13. Do not enter "See Plans" or "Refer to Narrative". Your application will be rejected.
  14. I, the applicant, understand my application will be rejected if I do not include a Detailed Scope of Work.*
    "Refer to Plans" or "Refer to attachment" will not be accepted in lieu of a detailed scope of work on the application describing my entire project and will cause my application to be rejected.

  17. If removing more than the trigger level (32 sq ft), attach a copy of the asbestos report.
  18. Plans must be scanned in on at least 11" x 17" paper. The application will be rejected with drawings scanned in on 8 1/2" x 11" paper. No exceptions.

    Plan review fees will be required in association with this application (building permit and other applicable fees are paid later in the process, at the time of permit issuance). For this permit, plan review fees may be submitted over the phone, in person, or via mail. Payment directions will be sent once this application has been received and reviewed for completeness.
