      YOU MUST ATTACH AN ELECTRONIC PAYMENT FORM IN ORDER FOR THE PERMIT TO BE PROCESSED.
      THIS APPLICATION DOES NOT CONSTITUTE A PERMIT. DO NOT BEGIN WORK UNTIL PERMIT HAS BEEN ISSUED.
